When people living with HIV are connected to care and take prescribed HIV medications, they can lower their viral load (or the amount of HIV present in their blood) so much that the virus is undetectable and cannot pass through sex. That means they can live long and healthy lives. It also protects the health of their sexual partners.

A Linkage to Care Specialist (LTCS) connects people with a new HIV-positive diagnosis or HIV-positive people currently not receiving care to the appropriate medical care that they need. They help clients understand what their status means for themselves and others, and to commit to a lifestyle that enhances their health and protects that of others.

Linkage To Care (LTC) is an ongoing process where professionals help people with a new HIV diagnosis or people living with HIV and currently out of care navigate the early stages of treatment.

If you are in need of any assistance from housing to health care, connect with a case manager. They can be your best advocate!
When you are dealing with an emergency, your case manager can direct you to the service providers who can help you. Case managers have experience dealing with landlords, health care providers, insurance companies, and medication support programs.
When you are feeling well and your living conditions are stable, your case manager will help you think through a plan to maintain your health and stability, deal with emergencies, and direct you to support services that you may need.

Ryan White State Drug Assistance Program (SDAP)
SDAP helps people pay for costly HIV medications. The program is open to Colorado residents living with HIV with an income less than 500% of the Federal Poverty Level.
Qualifications
- You are a Colorado resident.
- You are a person living with HIV/AIDS verifiable at CDPHE or from a doctor or testing facility.
- Your income is less than the 500% Federal Poverty Level.
- In 2023, the 500% FPL for a household of one is $72,900.
Coverage Guidelines
- SDAP is the payer of last resort. The system only accepts those with no other options.
- If you can receive assistance with Medicaid, you must verify that you have applied for that program. (You may receive temporary access to our uninsured program while this application is pending.)
- If you can receive insurance through your employer, a COBRA policy, or Connect for Health Colorado, you'll need to be enrolled in Colorado SDAP’s Health Insurance Assistance Program (HIAP).
- You'll receive assistance in paying the costs of enrolling in this insurance.
- If you're eligible for Medicare, you'll also need to join SDAP’s "Bridging the Gap Colorado" plan for help with the costs of prescription premiums and related co-payments.
- If you have Health First Colorado (Colorado's Medicaid program), the Supplemental Wrap-Around Program (SWAP) can help cover the copays on your SDAP formulary medicine.
- If you can't access any other insurance, you'll be enrolled in the HIV Medication Assistance Program (HMAP).
Renewals
- All members of Colorado SDAP must renew their eligibility for the program every six months.
- This recertification is due on the last day of your birth month and the last day of the month six months from then.
- Please submit your recertification documents on time to avoid delays in getting your prescription drugs or the loss of insurance. SDAP can't pay for premiums for people whose eligibility has expired.
